Health Insurance
- Limited open
enrollment – you
must enroll within 25 calendar days from your first day of work.
- Your coverage and premiums will begin on the first day
of the month following your first day of work.
- If
you wish to decline health insurance coverage or fail to return
your enrollment form within 25 calendar days, then you will
be ineligible to
elect coverage for yourself and/or dependents until the
next open enrollment period (March 2005) or other qualifying
event.
- The total monthly cost of your portion of the health insurance
premiums will be automatically deducted from your paycheck
in two equal installments
per month.
Dental Insurance
- Continuous
open enrollment – you may enroll
at any time.
- Your coverage and premiums will begin on the first day
of the month following receipt of your enrollment form.
- The total monthly cost of your portion of the dental
insurance premiums will be automatically deducted
from your paycheck
in two equal installments
per month.
Flexible Spending Account
- Limited open
enrollment – you must enroll
within 25 calendar days from your first day of work.
- Your contributions to the FSA program will begin on the next payroll
following receipt of your enrollment form.
- If you wish
to decline participation in the FSA program or fail to return your
enrollment form within 25 calendar days, then you will
be ineligible
to participate until the next open enrollment period (January
2005)
or other qualifying event.
401K Investment Plan
- Continuous
open enrollment – you may enroll
at any time.
- Your contributions to the 401k program will begin on the next payroll
following receipt of your enrollment form.
